Magnesium is involved in over 300 enzymatic reactions in the body, which is why it shows up in conversations about sleep, muscle recovery, heart rhythm, and stress. But the same mineral that eases a cramping muscle can also block a thyroid pill or cut an antibiotic's effectiveness in half if the timing is wrong.
Where Magnesium's Reputation Is Earned
The clearest mechanism-level case for magnesium is muscular and neurological. It functions as a natural counterweight to calcium, according to an overview of conditions that may benefit from magnesium, which lets muscles relax after calcium triggers them to contract — the basis for its use in easing post-workout twitching and cramping. The same balancing role extends to the heart, where magnesium helps coordinate the electrical signals behind each heartbeat and supports blood vessel relaxation tied to healthy blood pressure.
Sleep and stress are connected through the same pathway. Magnesium supports the nervous system and the production of melatonin, the hormone that regulates the sleep-wake cycle. Stress compounds the problem in the other direction: elevated cortisol increases magnesium loss through the kidneys, and lower magnesium then makes it harder for the body to regulate a stress response — a cycle rather than a single cause-and-effect event.
The Metabolic and Migraine Case

Magnesium's role in glycolysis and ATP production means it is directly involved in how cells generate usable energy, which is the biochemical basis for its association with metabolism and fatigue. Migraine prevention is the one area where the source material points to a specific, medically supervised dose rather than a general wellness claim: high-dose supplementation, often above 350 mg per day, is frequently prescribed for migraine prevention under a doctor's guidance rather than through open-ended self-supplementation.
When Extra Magnesium Becomes a Liability
Overdose is uncommon in people with healthy kidneys, because the kidneys flush out excess magnesium through urine. The risk concentrates almost entirely in one group: people with chronic kidney disease or other kidney impairment, whose bodies cannot clear the surplus. For this group, even moderate supplementation can accumulate to dangerous levels.
A separate, easy-to-miss risk comes from hidden sources rather than supplement bottles themselves. Over-the-counter laxatives and antacids can contain magnesium doses as high as 5,000 mg per day, far above what most supplement labels list, according to reporting on the risks of taking too much magnesium. Early signs of excess are usually mild — diarrhea is the most common immediate symptom as fluid is pulled into the bowel, along with nausea and stomach cramping. Left unaddressed in a high-risk person, symptoms can progress to muscle weakness, a slowed heart rate, dangerously low blood pressure, confusion, respiratory distress, and in extreme cases cardiac arrest.

The Medications Magnesium Can Quietly Undermine
The more common real-world problem isn't overdose — it's magnesium interfering with other drugs a person is already taking. Several interactions come down to the same chemistry: magnesium binds to certain drug molecules in the gut before the body can absorb them.
Antibiotics in the tetracycline and fluoroquinolone classes, such as Cipro and doxycycline, are the clearest example. Magnesium chelates to these drug molecules and can reduce antibiotic absorption by 50 to 90 percent, according to reporting on drugs to avoid combining with magnesium. The recommended workaround is timing, not avoidance: taking the antibiotic two hours before or four to six hours after a magnesium dose.
Bisphosphonates used for osteoporosis, like Fosamax, face a similar absorption block and require at least two hours of separation. Gabapentin is a narrower but higher-stakes case — magnesium can lower gabapentin levels by 20 to 40 percent, which matters because gabapentin is often prescribed to control nerve pain or seizures, making a two-hour spacing gap a meaningful safety step rather than a minor precaution. Levothyroxine, the standard thyroid medication, competes with magnesium for absorption and needs a four-hour gap to avoid destabilizing hormone levels.
Two interactions run in the opposite direction. Long-term use of proton pump inhibitors for acid reflux — more than a year — impairs magnesium absorption by 30 to 40 percent and is a frequent, underrecognized cause of magnesium deficiency rather than excess. Diuretics split by type: loop and thiazide diuretics push the kidneys to excrete more magnesium, contributing to deficiency, while potassium-sparing diuretics do the opposite and retain it, which is a separate consideration for anyone in the kidney-impairment risk group described above.
None of these interactions are reasons to avoid magnesium outright for most people. They're reasons to separate the dose from the medication by the hours noted above, and to flag long-term PPI or diuretic use — or any degree of kidney impairment — to a doctor or pharmacist before adding a magnesium supplement to a daily routine.
